- RESUME: Please attach a resume, limited to 2 pages in length. Suggested items to include on your resume are honors, extracurricular activities, memberships and offices held in organizations, accomplishments, and volunteer work.
- SHORT ESSAY: Please submit a 1-page, single spaced essay addressing this topic:
  - Please describe your past or current service to racial and ethnic communities, your future plans to positively impact and represent Utah's racial and ethnic communities, and describe how Utah's ethnic communities will benefit from your legal education.
- OPTIONAL (0-2) LETTERS OF RECOMMENDATIONS: You are invited to submit up to two letters of recommendation, 1 page per letter.

By submitting my application to UMBA for consideration, I am certifying that all information provided is complete and accurate to the best of my knowledge. I hereby give UMBA permission to use this information and my photograph for purposes of awarding and granting the UMBA Scholarship. Should I receive a UMBA Scholarship, I give UMBA permission to publish my name, law school affiliation, and photo on UMBA's website. I also represent that I will use any scholarship funds only toward expenses related to my college or professional school attendance. I acknowledge that after receiving the UMBA Scholarship, I am subject to audit to confirm that the funds were used as certified herein. Should I not use the funds consistent with this application, I understand that I shall be responsible to repay those funds to UMBA. I also understand that I must notify UMBA of any changes in my enrollment status which may result in the cancellation of any awards provided. Furthermore, any scholarship check from UMBA must be cashed or deposited within 90 days or the funds will revert to UMBA. I hereby certify that I have read the application information and instructions and that I understand and accept all conditions specified. Falsification of information may result in termination of any scholarship granted. All application materials become the property of UMBA.
